[clangd] Add RemoveUsingNamespace tweak.
Summary:
Removes the 'using namespace' under the cursor and qualifies all accesses in the current file.
E.g.:
using namespace std;
vector<int> foo(std::map<int, int>);
Would become:
std::vector<int> foo(std::map<int, int>);
