Add Triggering Logic for Payments Churned Users Bubble

This CL adds the triggering logic and tests for the payments churned
users bubble. The requirements to display the bubble this CL adheres to
are:

1) There is a visible credit card form on the page. 2) The user has
payments autofill turned off.
