Factor extensions out of GetAuthenticatorAssertionResponse. Before this change extension properties were flattened into GetAuthenticatorAssertionResponse, now AuthenticationExtensionsClientOutputs holds these extension properties. All references (read/write) are updated to be accessed through the new extensions property on GetAuthenticatorAssertionResponse, and the new extensions property is initialized at each site where GetAuthenticatorAssertionResponse is itself initialized. For historical reasons, there are some small top level directories. Now the guidance is that new top level directories are for product (e.g. Chrome, Android WebView, Ash). Even if these products have multiple executables, the code should be in subdirectories of the product.
