Add a warning when "already started" speculation rules are inserted. This happens most notably when the innerHTML setter is used to insert them, because script elements created by fragment parsing start already started. This is a little broader than that, though. For historical reasons, there are some small top level directories. Now the guidance is that new top level directories are for product (e.g. Chrome, Android WebView, Ash). Even if these products have multiple executables, the code should be in subdirectories of the product.
