[DNS] Report FP interactions and FP separately

Report interactions with revoked or proposed revoked sites. Only report
false positive revocation when all conditions (min SES delta, after
cooldown period) are satisfied, include the interaction counts that have
occurred during the cooldown period (page visit count and notification
click count).

After adding new fields to RevocationEntry, it became a complex type and
it has to have constructors and cannot use initializer lists.
